An instructor, departmental secretary, or student assistant may obtain sheets at our office. Sheets are not distributed to departments. There are several different types of sheets available.

Because the costs of Opscan sheets are high, please do not throw away unused sheets. Either save them for your own use at a later time or return them to the Opscan Evaluation office.
You will notice on the older sheets a field called “Social Security Number.” On newer sheets, this has been replaced with a field called “University ID.” Regardless of which sheet you use, students should never enter their Social Security Numbers (SSNs) on the sheets. The university is actively trying to avoid identity theft by greatly limiting the use of SSNs.
For most situations, students should enter their university identification number (UID) in the field. For certain reports (scores for WebCT and Grade Book), students must enter their UIDs. However, if you would like to post grades in a classroom, then you should assign a random number to each student that is less than eight digits and ask the students to enter that custom identification number instead in the “University ID” field.
